Getting Ready

When you come to your appointment, don't forget to bring these items:

• Driver's License or Photo Identification Card
• All W-2 Forms
• 1099 or 1098 Forms (if any)
• Social Security Cards or Individual Taxpayer Identification Number (ITIN) for all persons in your family
• Any tax statements received from your mortgage company during 2011
• Last year's tax return, if available
